Sorry it's a long one, I rather like writing in character. Here is the profile of the warband, I've just got to make the models now!

Physical Appearance: Normally wearing the ubiquitous Commissars long coat and hat. Grave appearance, often with facial hair. Normally wears an ornate cuirass. This was a gift from the survivors of [+DATA LINK ERROR+]. Has a long scar down one side of his face, and a bionic right eye.

Personality Assessment: A-7.4: Worrying Stable. Subject had demonstrated extreme inflexibility to behaviours outside the tenants of the Imperial Creed. This could because of war-time experiences in [+DATA LINK ERROR+]. The few survivors of this conflict would have a wealth of experience in areas of interest to the Ordos Malleus. They would also most likely be insane. It is highly likely to be the case for the subject. It is possible that his insanity is demonstrated as extreme belief in the Imperial Creed. A further factor in this obsessive behaviour is the specific, accurate and extensive paperwork which the subject creates and covets. Is a committed believer in the Amalathain Doctrine. However, the behaviour of the subject in all other factors demonstrates A-9 high echelon human level higher reasoning and cognition.  Subject demonstrates no psychic ability, but it has been observed resisting psychic attacks.

Penal Guardsman TX-7445-998 Uusa Wainbridge – Believed to be the other survivor of [+DATA LINK ERROR]. Convicted of wasting ammunition. Sentenced to seek redemption through combat. Has been observed wearing the penal uniform with bomb necklace attached. The reason for the inclusion on this man in Io's associates is unknown. Io could be allowing Uusa to seek the Emperors redemption through an honourable death. Personality rated as B-5, but with additional desperation complex. 

Scribe Unit 947-Alpha-886 'SUA' – Scribe servitor, seen to accompany Io wherever he goes. Specifically developed to create and fill out paperwork for Io's own specially designed bureaucracy. Low level cognitor unit, no definable personality.

[Unknown first name] Kell – Heavy weapons specialist. Background unknown. Intriguing relationship between this woman and Io considering she was not involved in [+DATA LINK ERROR+]. Seen accompanying Io since Ronex V incident. Believed to have some genetic or biological enhancements due to size of heavy weapon. Training and background unknown, speculated as gang-based. Once heard to curse the Imperial Creed in the area of Io. Treatment after this is unknown. Personality assessment marginal B class and demonstrates obsessive tendencies.

Drill Abbot Jute Fu'ddo – Personality previously assessed to A6/A7 margin. Fu'ddo has extensive combat experience. Service includes Armageddon, the Reclamation of St. Jowens Dock and Sabbat Worlds. Commended for service in defence of Vervunhive. Faced daemons on Aexe Cardinal. Assigned to Schola Progenium to teach in M.41.985. Trained Io's class of commissars. Logs recovered from the Schola support his strong faith and endearment in Io. Vanished from the Schola in M41.999.9. Officially AWOL. Believed now to be some sort of 'spiritual guide' and mentor figure for Io.

Combat Interdiction Engine 879993 'CIE' – Believed to once have been Brevet Colonel (Warrant Officer 2nd Class) Jam Readon, veteran of [+DATA LINK ERROR+]. However, Readon was officially recorded as [+DATA LINK ERROR+] since [+DATA LINK ERROR+]. It is believed the corpse was placed into stasis until it could be rebuilt. Combat Servitor unit has little personality. It has been seen to follow a series of set programmes with little autonomy.         

Sgt Pip Piperson- Pip is a Halfling scout/sniper, service number: 8855-3331-2299. Recruited to Grunderlan VXIII. Served with valour in the battle for Medusa V, Nocturne and Regula VII. Cited for bravery. Officially recorded as AWOL after garrison duty on Ronex V. Seen in the service of Io. Pip is understood to serve a scouting and investigation role. He is also believed to be the cook.

Overall Danger Assessment: High. Io is deeply inflexible, therefore Conservatives may find him a useful ally in their fights. However, anyone who demonstrates the slightest radicalism will be threatened. The inclusion of Fu'ddo may mean that Io is beginning to stretch his definition of the Imperial Creed. His attitude may be changing, but since it could be linked to his overall mental condition, speculation is unwise.

Special Rule for Inquisitor Io:

Due to his horrendous experiences, Inquisitor Io has developed an obsession with paperwork and its accuracy. The fact he has designed an entire bureaucracy to his specific needs demonstrates this. At all time, Scribe Unit 'SUA' must accompany the Inquisitor as closely as possible, with no other actions available to the servitor.

Secondly, in the case of Io only rolling a single action in his turn, his obsession gets the better of him. This single action will be to order the documentation he wishes to have completed, so filled with talk such as:

'Scribe! Attend to the B-4756 form, worthy expenditure of the Emperors ammunition. Follow this in triplicate with an A-317 form with the race and type of enemy engaged. Ensure that the action report has been completed up to the second, and make sure we have filled out form C-368, mitigation of collateral property damage...' 

Should' SUA' be destroyed or disabled, the Commissar will be so enraged he classes as having the Furious Assault skill for the rest of the engagement.

It looks pretty good. I'd normally object to the Nerves of Steel/Force of Will combo (being more indicative of disinterest in one's well-being than bravery), but I guess an insane Malleus ex-Commissar fits that bill.

One problem though:

While characterful, that's going to absolutely kill his efficiency. For a speed 4 character, they'll only get a single action in 5/16ths (31%) of turns.

Don't forget, IGT games are tight for time, so I'd probably get a little frustrated if I was GMing and your time spent declaring actions was completely wasted almost one time out of three.
So, I'd tone it down a bit, making it so that it only happens if he rolls no successful 4+ action dice, at which point he uses his "guaranteed" action for the purposes of paperwork. This reduces the chance to 6%, making it much less of a delay on gameplay - but not so rare that it will never happen.

Whilst I like the idea of a paperwork-obsessed inquisitor, I agree that doing nothing if he rolls a single action feels excessively harsh, and even in an unstable character, I would like to see it toned down a bit. Possibly by allowing it to be combined with a movement action. Also, it seems like something that should be subject to his immediate situation-pausing to record things during a moment of quiet isn't so bad, but is he really deranged enough to stand in the middle of a firefight, or even close combat, saying things like

"Scribe, record that a heretic has a boltgun aimed at my head, and seems about to open fire."

Actually, that is kinda what I imagined when I designed the character! This desire is something like OCD, a compulsion which he has to fight all the time. So when he can't resist, despite the bullets raining about him, the paperwork becomes the most important thing in the world, bar nothing. He really is that deranged!

But all the comments are appreciated, especially with the rules suggestion. I think that I might modify it to:

'When commissar Io rolls no successful actions, he is overtaken by the desire to fill out the paperwork. The only action available in this case is to make one walk action in any direction, while addressing his servitor thus:

'Scribe! Attend to the B-4756 form, worthy expenditure of the Emperors ammunition. Make sure this is filled out in triplicate and marked with my seal. Make a note of the time with the firing, and update the log...'

However, if he is in close combat, this rule is ignored. Combat is suitable distracting for Io already
